NY Medical Marijuana CertificationTo obtain your NY medical cannabis card, you must (1) have a recent medical document clearly stating any of the above conditions within the past 12 months, (i.e. chronic pain, back pain, joint pain, etc) and (2) have tried another intervention (massage therapy/ chiropractics/ physical therapy, oral analgesics, topical agents) for three months without relief. Because each situation is unique, our providers make a strong effort to determine whether patients meet diagnostic criteria using alternative examples of qualifying documents by analyzing your information: physical therapy, radiographic imagining, psychiatrist/ PCP/ specialist’s notes, or prescription records of opioid long-term use to name a few. Once you gather this information, kindly submit your documents to our HIPAA compatible portal so our doctors can evaluate your file and better assist you. * Note: If your diagnosis is not within the last 12 months, feel free to contact your provider and update your records.

NYS Qualifying Conditions:

  • Chronic pain
  • Cancer
  • Cachexia or wasting syndrome
  • HIV+/AIDS
  • Amyotrophic Lateral Sclerosis
  • Parkinson’s Disease
  • Multiple Sclerosis
  • Huntington’s Disease
  • Spinal cord injury with any of the following:
    • Muscular Spasticity
    • Neuropathic Pain
  • Epilepsy
  • Inflammatory Bowel Disease (IBS)

Medical Clearance:

For those patients that meet diagnostic criteria, we upload your information to the NY State Medical Cannabis Program to receive a 12-month medical cannabis card that can be taken to any NY medical marijuana dispensary. To receive your card, each patient shall submit a passport style photo (headshot). To do so, please submit your photo along with your medical records on the secure link along with a copy of your medical records. Additionally, we encourage our patients to set up a six-month follow up, so that we can determine how your treatment plan is going and whether any modifications need to be made.
